______ I understand a $20 postage deposit is required to start an account with Cleveland Redirect Service. I further understand that a strict accounting of the postage money is kept, and that I will be notified when my account goes low.

______ I hereby agree to conduct my business in an honest and ethical manner. I also agree to abide by the provisions of the US Postal Regulations and Cleveland Redirect Service. I understand that Cleveland Redirect Service will not knowingly handle any illegal mailings through this re-mailing service.

______ I agree that Cleveland Redirect Service is not responsible for any violations on my part and agree to re-imburse them for any time and legal expense involved.

______ Upon termination of service or failure to pay rent in advance by applicant, Cleveland Redirect Service shall have no responsibility to make applicant's mail available without payment therefore. Applicant understands that the US Postal Service will not forward mail without payment, will not return mail to sender and will not accept change of address. If applicant wished to receive any mail received for him after date of termination, he shall provide Cleveland Redirect Service with a forwarding address and pay the required fees for mail forwarding. In the event applicant fails to do this, My Branch Office shall refuse further mail, and in the case of mail already received, retain such mail for not more than ninety (90) days, after which is will be destroyed or returned to the Post Office. All services are payable in advance.
